Volunteer forms

    To volunteer at River City Clinic all volunteers complete and submit the HIPPAA form and
    complete the quiz at the end.  HIPPAA helps us to ensure that patient privacy is
    protected.  We feel very strongly about protecting patients and their information.  
    Violators of the below described HIPPAA regulations will not be allowed to volunteer at the
    clinic.  If you have any questions about this please don't hesitate to ask!!!

    Also we would like you to complete the dress code form.
